Join us for the next Readmission Care Partner sprint webinar, which will cover the fourth step of the EQIC Care Partner framework, preparing the care partner for the next transition.
The featured is speaker Brenda Chapman, BS, RNC from HANYS. Chapman is a readmission, discharge planning and care transitions subject matter expert and will provide best practices, suggestions and practical details on how to operationalize elements of this implementation step for preparing the patient and care partner for the next care transition.
Hospitals have a full syllabus, detailed steps, homework and access to tools and resources to develop and hardwire workflows utilized in transforming into a care partner hospital. EQIC project managers will assist hospitals progressing through their action plans, including training staff members on key content from this webinar.
EQIC encourages participation of key team members, including chief nursing officers, chief medical officers, patient experience leads, nursing and pharmacy staff, hospitalists, care transition team and other clinical and professional staff members.
