Readmission Care Partner sprint: Capstone - Recording

EQIC is wrapping up its Readmission Care Partner sprint with a capstone webinar. Participating hospitals will share their experiences and successes in implementing the four-phase framework of the Care Partner Program.

During the sprint, hospitals learned about the care partner approach, including best practice and evidence that gives staff the structure, parameters and tools to facilitate successful patient-centered care throughout the hospital course and discharge transition. Executed well, evidence shows this care delivery model can have a significant impact on decreasing readmissions and improving patient satisfaction.

EQIC is proud of the work done by all participating hospitals throughout this sprint to adopt the framework and implement a care partner program. Hospitals that complete the Care Partner Implementation Checklist can work with their project manager to submit an Attestation and earn a Care Partner Hospital designation from EQIC to show their community their commitment and dedication to improving care and the patient experience.
