Medication management and discharge planning - Recording

Drs. Felicia Mendelsohn Curanaj and Jane Jeffrie Seley of NewYork-Presbyterian Weill Cornell Medical Center will lead the final session in the EQIC inpatient glycemic management sprint.

This presentation will review medication interactions and considerations for ADE; identifying high-risk medications; care team considerations to reduce ADE; considerations for discharge planning and outpatient follow up; and guidance on patient self-education and management of insulin.

Providers and care team members are encouraged to attend.
