Interventions for the MAP program - Recording

EQIC’s next Multiple-Admission Patient program session will focus on step four of the MAP framework: customizing interventions. To address the unique needs of each MAP, your team will develop patient-specific interventions based on feedback from the patient and care partner interview, readmission risk assessment and MAP data analysis.

EQIC staff will provide strategies for the MAP team to mitigate risks identified for frequent admission and readmissions, including:

• customizing individual interventions and discharge plans;
• engaging the emergency department staff;
• ensuring communication with post-discharge providers; and
• providing post-discharge support and follow-up.

EQIC encourages key leadership team members, including your chief medical officer, chief nursing officer, patient experience officer, care or case management members and social workers to participate. Nursing staff, hospitalists, ED staff, MAP team members and other clinical and professional staff are also encouraged to attend.
