EQIC preventable readmission measures seek to assess patients who are readmitted to the hospital and why. This includes reviewing measures that break down readmissions by race and specific disease states.
EQIC produces two readmission-specific special reports. The MAP report analyzes which patients are admitted to the hospital four or more times in the calendar year. It is built off of the AHRQ data analysis tool and can be replicated by hospitals for continued assessment of MAPs. This hospital-specific report identifies areas of opportunity to engage patients who are readmitted multiple times per year by exploring the reasons why they return to the hospital. This report can be used in conjunction with the MAP sprint materials.
The Readmission among patients with and without diabetes report stratifies the top 10 readmission diagnoses and then further stratifies the report by patients with and without diabetes.
