Readmissions Care Partner sprint: Preparing for attestation office hour - Recording

This session in EQIC’s Readmission Care Partner sprint series is a virtual office hour, which will include an opportunity for hospitals to share challenges and successes with hospital colleagues. Additionally, EQIC faculty will offer ideas on best practice strategies for adopting the care partner model and review criteria for earning a Care Partner Hospital certificate.
The open forum allows for interactive discussion, and attendees are encouraged to submit questions or topics in advance to EQIC@hanys.org.
