The impact of health disparities and social determinants of health on readmission - Recording

EQIC’s third webinar in the Multiple-Admission Patient program series focuses on health equity and the role of social determinants of health in reducing disparities in readmissions. SDOH are the conditions in which people are born, live, learn, work, play and worship, and they affect a wide range of health, functioning and quality-of-life outcomes and risks.

Building upon step three of the MAP program framework –– assess readmission risk –– participants will learn key strategies for building and strengthening effective community partnerships. By working together to align organizational priorities with your community’s needs, hospitals can use data-driven strategies to craft creative solutions to address and mitigate the root causes of health inequities.

By the end of the session, attendees will understand how to use data tools and qualitative metrics to identify the SDOH most commonly found within your specific patient populations and take appropriate action to improve patient care.

EQIC encourages key leadership to participate, including chief medical officers, chief nursing officers, patient experience officers, care or case management members and social workers. Nursing staff, hospitalists and other clinical and professional staff are also encouraged to attend.
