Role of the emergency department — 15 years of case management: Lessons learned and benefits realized - Recording

The next session of EQIC’s Multiple-Admission Patient Program sprint will focus on strategies to engage emergency department staff as part of the MAP team. ED staff are in a unique position to proactively identify patients who may be readmitted and can determine appropriate strategies to support high-risk patients.

Dr. Casey Grover will provide insight and benefits for implementing case management and engaging other MAP team members in the emergency room to identify interventions aimed at reducing admission or readmission.

EQIC encourages key leadership team members, including your chief medical officer, chief nursing officer, patient experience officer, care or case management members and social work staff to attend. Nursing staff, hospitalists, ED staff and MAP team members are also invited to participate.
