QIC’s health equity work is designed to assist hospitals: build and sustain an organizational response to eliminating disparities and achieving health equity; make health literacy and cultural competency an institutional priority; and address social determinants of health and prevent readmissions.
This workgroup provides a platform for hospitals to develop action plans to improve performance outcomes to meet these goals. Action plans will explore various opportunities for improvement to facilitate hospitals’ ability to take action and implement tools, resources and workflows to improve performance outcomes.
