EQIC’s readmissions work is designed to assist hospitals with implementing strategies to identify patients at risk for preventable readmissions and improve care transitions, including health equity implications, with the goal of reducing readmissions by 5%.
This workgroup provides a platform for hospitals to develop action plans to improve performance outcomes to meet these goals. Action plans will explore various opportunities for improvement to facilitate hospitals’ ability to take action and implement tools, resources and workflows to improve performance outcomes.
