Our second Readmission Care Partner sprint webinar will cover the first two steps of the EQIC Care Partner framework, Commit and Identify. The featured speaker is Maria Sacco, RRT, CPHQ, Director, Quality Advocacy, Research and Innovation at the Healthcare Association of New York State. Ms. Sacco has worked closely with advisor Dr. Amy Boutwell over the last three years developing curriculum, teaching, mentoring and providing hospitals with guidance in establishing care partner programs. She is a readmission, discharge planning and care transitions subject matter expert and will provide best practices, suggestions and practical details on how to operationalize elements within these two implementation steps.
The hospitals/attendees will receive the full syllabus, detailed steps, homework and access to tools and resources to develop and hard-wire workflows utilized in transforming into a care partner hospital. EQIC project managers will assist hospitals progressing through their action plans, including training staff members on key content from this webinar.
EQIC encourages participation of key team members, including chief nursing officers, chief medical officers, patient experience leads, nursing and pharmacy staff, hospitalists, care transition team and other clinical and professional staff members.
