Join us for the next Readmission Care Partner sprint webinar, which will cover the third step of the EQIC Care Partner framework, Include.
The featured speakers are Michelle Van De Graaff RN, BSN from Intermountain Medical Center and Nancy Landor, RN, MS, CPHQ, from HANYS. Van De Graaff will discuss the Intermountain Partners In Healing® program, which prepares patients and families to manage healthcare at home by involving care partners with the healthcare team. Landor will provide best practices and suggestions on how to operationalize elements of this implementation step for including the care partner as a member of the healthcare team. Hospitals will receive the full syllabus, detailed steps, homework and access to tools and resources to develop and hardwire workflows utilized in transforming into a care partner hospital. EQIC project managers will assist hospitals progressing through their action plans, including training staff members on key content from this webinar.
EQIC encourages participation of key team members, including chief nursing officers, chief medical officers, patient experience leads, nursing and pharmacy staff, hospitalists, care transition team and other clinical and professional staff members.
