Join EQIC for the next Readmission Care Partner sprint webinar, which covers transitions of care and the inclusion of community partners.
The featured speakers are Gale Grunert from Lewis County General Hospital and Maria Sacco and Brenda Chapman from HANYS. EQIC faculty will provide an overview of the Care Partner program with the goal of increasing community-based organizations’ involvement in the Care Partner model. Grunert will discuss their hospital’s Care Transition Community Collaborative developed with the assistance of IPRO’s QIO, which includes the identification and involvement of community-based organizations. The Community Collaborative works together to identify patients at risk for readmissions and cooperatively mitigates those risks in coordination with the patient and care partner.
EQIC encourages participation of key team members, including chief nursing officers, chief medical officers, patient experience leads, nursing and pharmacy staff, hospitalists, care transition team members and staff from community based organizations, such as primary care, skilled nursing facilities, home health, behavioral health, social service organizations and rehabilitation providers.
Staff from hospitals and community-based organizations that participate with a CMS Quality Improvement Organization or another Hospital Quality Improvement Contractor like EQIC also are invited to attend. We are proud to share our Care Partner Model and foster collaboration among fellow HQICs and QIOs to help improve healthcare outcomes nationwide.
