EQIC is pleased to announce its new CAHort and invites you to attend the inaugural meeting. The CAHort is a collaborative effort with other critical access hospitals in the program to achieve improvement goals focused on your organization’s needs. CAHs face unique challenges due to availability of resources and the patient populations served. Collaboration with similar organizations is crucial to driving improvement in this setting.
Through virtual meetings, you will engage with other staff from similar hospitals to address shared challenges, improve quality metrics and strategize opportunities for improvement. The CAHort is an opportunity for networking and data sharing to address the common challenges faced and opportunities available for improving quality outcomes and collectively reduce hospital-acquired conditions.
During this introductory session, EQIC staff will review the purpose and structure of the CAHort and identify goals and priorities. We suggest the participation of the following staff for this interactive working meeting:
Background
EQIC was chosen by CMS to deliver services for its Hospital Quality Improvement Contract to further advance the quality and safety of healthcare provided in hospitals across the region. Notably, CMS targeted for participation rural and critical access hospitals. EQIC’s CAHort allows us to assist these member hospitals to achieve the overarching CMS goals of reducing all-cause patient harm by 9%, readmissions by 5%, adverse drug events by 13% and adverse drug events related to opioids by 7%.
EQIC will work within the CAHort to focus on the most pressing issues for those patient populations within the framework of CMS’ overall improvement goals to ensure your hospital addresses the areas of most need. Patient and family advisory councils, readmissions and opioids will be the focus areas covered within the CAHort with workgroups formed in each of these areas based on interest. Additionally, the importance of achieving health equity within each of these areas will be emphasized.
Participation in the CAHort consists of quarterly webinars to review priorities and network with similar organizations. Between meetings, your team will work with your project manager and within your self-selected workgroup(s) to achieve your hospital-specific goals.
