EQIC is pleased to host the first webinar of its Multiple-admission Patient program, which presents best practices for identifying patients frequently admitted to the hospital. The MAP program focuses on engaging patients, designated care partners and community-based organizations to identify, manage and address transitional needs across the continuum of care to reduce readmission.
EQIC defines MAPs as individuals with four or more hospital admissions in a 12-month period. Using the EQIC MAP program framework, your team will learn to identify MAPs using data-driven reports and reduce hospital admissions by meeting patient needs at home or the next level of care. Collaboration among hospitals and CBOs helps address the clinical, behavioral and social needs of high-risk patients to prevent repeat hospitalization.
After this session, attendees will understand the value of the MAP program and how to identify MAPs and take a community-based approach to reduce readmissions. Strategies include building EMR notification for MAPs, developing internal staff education, planning MAP program team communication and regular meetings and developing a mechanism for patient-specific communication among engaged CBOs.
EQIC encourages key leadership team members to participate, including chief medical officers, chief nursing officers, patient experience officers, care or case management members and social workers. Nursing staff, hospitalists and other clinical and professional staff are also encouraged to attend.
