Assess patients at risk for multiple admissions and readmission - Recording

The next session of the Multiple-admission Patient Program sprint will review the process and procedures for identifying a patient’s risk factors for admission and preventable readmission. Risk factors can be addressed by using a standardized risk assessment tool, interviewing the patient and care partner and linking the patient to community-based organizations for appropriate post-hospital follow-up and services.

EQIC will provide strategies for identifying patient risks for preventable readmission, developing a patient-specific supportive plan for transitioning care needs and working with the patient’s care partner and MAP team to mitigate the identified risks.

EQIC encourages key team members, including chief nursing officers, chief medical officers, patient experience leads, nursing and pharmacy staff, hospitalists, care transition team and other clinical and professional staff members to participate.
