EQIC is pleased to present our “Reducing Hospital Readmissions by Partnering with Skilled Nursing Homes” initiative. During this two-hour caucus, EQIC will provide best practices for improving the patient and care partner experience across care transitions by implementing workflows and tools to strengthen communication between facilities.
EQIC encourages hospitals to partner with SNFs that have frequent readmissions to your facility. Through a collaborative work approach, hospital and SNF teams will gain an understanding of four key strategies to reduce readmission rates:
We are thrilled to welcome Sara Butterfield and Diane Judson from IPRO to moderate a panel-type discussion with St. Mary’s Healthcare, Wilkinson Residential Health Care Facility, St. Peter’s Hospital and St. Peter’s Health Partners, teams that collaborated as part of IPRO’s program.
EQIC recommends that hospital and SNF quality management leads, clinical and ED managers, nursing directors; discharge planners and readmission team members participate. Additional SNF staff such as admission coordinators, nurse managers, frontline staff, providers, pharmacists and rehabilitation staff are also encouraged to attend.
