Chapman is a readmission, discharge planning and care transitions subject matter expert and will provide best practices, suggestions and practical details on how to operationalize elements of this implementation step for preparing the patient and care partner for the next care transition.
Hospitals have a full syllabus, detailed steps, homework and access to tools and resources to develop and hardwire workflows utilized in transforming into a care partner hospital. EQIC project managers will assist hospitals progressing through their action plans, including training staff members on key content from this webinar.
